If you or your dependents completed the HealthQuest survey in calendar year 2008 and received a gift card, you will see an additional earn code for the pay period ending Nov. 1, 2008. The gift card is considered taxable income, requiring additional taxes to be withheld. The amount is listed in the earnings column of your pay advice.
