J-1 Exchange Visitor Check-In

Upon receipt of your completed check-in form, the Office of International Education will validate your DS-2019. The purpose of validation is to verify for Immigration and Customs Enforcement (ICE) that you have reported to Wichita State. You must submit this form no later than Friday, June 12. You will be required to upload three items when submitting your check-in form:

J-1 Visa
DS-2019. Please upload page one of the DS-2019 that was signed by the consular officer.
I-94 Arrival Record. The I-94 is a record of your most recent arrival in the U.S. Visit the CBP website, select “Get My Most Recent I-94”, enter your details, and then save your result to upload to this form.
