The Graduate School has provided a compilation of forms that are both important and in many cases, required, for undergraduate and graduate students. Undergraduate students that are interested in applying to the graduate program in Psychology must submit an Application for Admission to the Graduate School.

The following are important forms for current graduate students:

  • Degree completion forms - includes the required plan of study, dissertation proposal design, and application for degree/exit survey.
  • Exception forms - Departures from the rules and regulations stated in the Graduate Catalog require the filing and approval of a Request for Exception to Graduate School Regulations form.
  • Student travel fellowship - scholarships to students who present the results of their research at professional meetings and conferences.
  • Graduate school awards - grants and awards for which students and faculty may apply.
