Graduate Nursing Program Clearance for Enrollment Request Form

Please complete the short form below to request clearance to enroll. Allow a minimum of 2 business days for your request to be processed before enrolling yourself in the courses submitted.

Requests will be completed if there is space remaining in the course. You WILL NOT be contacted unless there is a question about your request. If you are enrolling in a practicum, your required practicum documentation must also be current. An explanation of those requirements is provided below for reference.
