The use of inflatable equipment on University premises must be in compliance with WSU Policy 11.13 / Use of Inflatables on University Premises and all applicable state laws. No WSU employee and/or leader is authorized to sign an agreement for the use of an inflatable equipment without the prior approval of the Office of General Counsel.

Requests to use inflatables must be submitted on the Request to Use Inflatables Form, and the following documentation must be provided along with the request:

  1. The University Inflatables Services Agreement signed by Vendor/Contractor
    • We recommend that you download this fillable PDF form and complete it before starting the online inflatables form. The online form will have a space for you to upload the agreement form.
  2. Vendor/Contractor's operation permit issued by the Department of Labor
  3. Current certificates of training for the inflatable equipment operators
  4. Current certificates of inspection for the inflatable equipment that will be used at the event
  5. Vendor/Contractor's proof of insurance, as required under the University Inflatables Services Agreement

To ensure that a request to use inflatables is approved in time for the scheduled event, requests should be submitted three weeks in advance.

Who can submit this form?

Only University employees are permitted to submit a Request to Use Inflatables Form. Student Organizations should contact the Office of Student Engagement, Advocacy and Leadership for further information on the process for requesting such use. Vendors should contact the employee or department that is hosting the event at which the inflatable will be used to complete the Request to Use Inflatables Form.

Waiver form

Any individual that will be participating in the use of inflatable equipment on University premises shall be required to sign the Wichita State University Release, Waiver, and Agreement Not to Sue for inflatables.
