Mailing Your OPT Application to USCIS

After reviewing your documents and recommending OPT in SEVIS, ISS will provide you with a new I-20 with OPT recommendation, a cover letter, and the SEVIS Employment Information Screen.  You are responsible for mailing your completed OPT application packet to the USCIS service center. 

Important notes:

• You will need to sign at the bottom of page 1 of the new Form I-20. After signing, make a copy of the I-20. You will keep the original for your records and send the copy with your application to USCIS.
• Don’t forget to include your Social Security Number (if you have one) on question 13. b. of the Form I-765.
• The OPT application must reach the service center within 30 days of the Designated School Official (DSO) entering the OPT recommendation into SEVIS.
