Proctor Request Form

This form is for students who are currently enrolled in courses at Wichita State University and are seeking a distance testing center. If you are unable to take an exam in the classroom or within the Office of Student Accommodations and Testing (OSAT), the submission of this form will initiate the approval process to use a proctored testing location near you.

If you are located in Wichita, you will need to schedule your appointment through OSAT using Register Blast.

IMPORTANT PROCTORING INFORMATION

All exams must be taken under the supervision of an approved proctor. The proctor may not be a personal friend, family member, tutor, academic advisor, coworker, or another student.

When a proctor is verified, all tests and testing information will be sent directly to the proctor's professional email address with proctoring guidelines. Exams will not be sent to any personal email addresses. NO EXCEPTIONS.

The distance location must be a testing center within a college or university. In rare occasions, advanced libraries may be approved if they provide professional testing options.

STUDENT RESPONSIBILITIES:

1. Find a valid testing site near your location
2. Contact the distance location to confirm whether they allow external testers
3. Submit this Proctor Request Form
4. Schedule your testing appointment and pay any associated costs at the distance location

Students who are living, working, and/or staying outside of the United States must locate their own proctor and these proctors must still meet the aforementioned requirements.

Upon clicking submit, your information will be sent to the Office of Student Accommodations and Testing for review. If the distance center is verified, OSAT will send a request for testing information to your instructor. After the instructor sends OSAT all of the testing information, the distance location will receive an email from testingcenter@wichita.edu. If the distance center is not verified, you must search for a different location and restart the request process.
