Proctor Request Form

Students who are currently enrolled at Wichita State University and are unable to take an exam in both the classroom setting and the Office of Student Accommodations & Testing will need to submit the following form to obtain approval to use a proctored testing location close to where they live.

Important Proctoring Information

All exams must be taken under the supervision of an approved proctor. The proctor may not be a personal friend, family member, tutor coworker, or another student.

When a proctor is verified, all tests and testing information will be sent directly to the proctor's professional email address with proctoring guidelines. Exams will not be sent to any personal email addresses. NO EXCEPTIONS.

If a professional proctored testing location cannot be identified, a local public library can be used as an alternative testing location.

It is the student's responsibility to find a proctor and pay any associated costs for testing. Some qualified testing proctors have been established by Wichita State University, please check with the Office of Student Accommodations & Testing to see if a proctor is set up in your area. It is also the student's responsibility to contact the proctor and set up appointments to take exams.

Students who are living, working, and/or staying outside of the United States must locate their own proctor and these proctors must still meet the aforementioned requirements.

The following demographic information will help us with the process in obtaining approval for you to take a distance exam at a proctored testing location within your proximity.
