Purpose

This template is intended for laboratory operations involving Particularly Hazardous Substances (PHS), including:

• Select carcinogens
• Reproductive toxins
• Substances with high acute toxicity
• Highly reactive or explosive chemicals
• Pyrophoric materials
• Other chemicals requiring special handling procedures

This SOP shall be completed by the Principal Investigator (PI), laboratory supervisor, or designated laboratory personnel prior to use of the substance.
