As part of your FAFSA's verification process, you must complete a Statement of Educational Purpose and submit a valid, government-issued photo ID to the Office of Financial Aid. Your signature on the form must be witnessed by a WSU Office of Financial Aid staff member - OR - witnessed and authorized by a Notary Public.

What is Needed
You have two options to complete the Statement of Educational Purpose form.
Option 1: Complete the form in person in the Office of Financial Aid in 203 Jardine
Hall, Wichita State University.
What to Do:
- Bring your valid, government-issued photo ID to the Office of Financial Aid in 203 Jardine Hall, Wichita State University.
- Financial Aid Staff will provide you the Statement of Educational Purpose form and witness your signature after verifying your ID.
Option 2: Complete the form in front of a Notary Public and submit original to the
Office of Financial Aid.
What to Do:
- Complete the form below in front of a Notary Public (charges may apply).
- Submit the original, signed form to the Office of Financial Aid via mail:
