The following form is for instructional staff to request that the Instructional Design and Access team conduct an evaluation of a piece of publisher or vendor provided software. The review will evaluate the relative accessibility of the tool based on information provided in the product's Voluntary Product Accessibility Template (or VPAT) and a review of the tool's functionality, if necessary.
